One team, one shared plan

Conditions we support

From gestational, type 1 and type 2 diabetes to heart health, weight, back and knee pain, nerve pain and foot problems, cared for across our Perth clinics. Pick your condition to see what to look out for and how our team helps.

One team for the conditions you live with

Most conditions need more than one kind of help. Our Credentialled Diabetes Educators, dietitians, exercise physiologists, physiotherapists, podiatrists and pharmacists work from one shared plan with your GP, so you tell your story once and every appointment moves in the same direction.

Find your condition

These are the conditions we see most often, listed A to Z. Search by name, or filter by the team you want to see. This is not a complete list and it is not a limit: plenty of people come to us with something that is not named here, or with several of these at once, and most are helped by more than one of our teams. If you cannot find what you are looking for, call (08) 6110 0570 and we will tell you whether we can help. Each page explains the condition in plain language, lists the signs worth noticing, shows which of our teams is involved, and is referenced to current Australian guidance.

Common questions

Plain answers about referrals, funding and where to start.

Do I need a referral?
+

Not for private appointments. For Medicare rebates, your GP can prepare a GP Chronic Condition Management Plan and refer you. Most people can then claim up to five eligible allied health visits a calendar year across our disciplines.

Which clinician should I see first?
+

Start with the condition page above: it shows which of our teams usually helps. If you are still unsure, call (08) 6110 0570 and our care team will book you with the right clinician first.

Can I see more than one clinician?
+

Yes, and many people do. Our clinicians work from one shared plan and keep your GP updated, so you do not repeat your story at each appointment.

My condition is not listed. Can you still help?
+

Very likely. These pages cover the conditions we see most often, not everything we treat. Browse our services or call us and we will tell you whether we are the right fit.

Is telehealth available?
+

Yes. Many of our services are available by phone or video where it is clinically appropriate.

Not sure where to start?

Tell us what is going on and we will point you to the right discipline, or to more than one. Most people start with a single appointment and leave with a plan the rest of the team can see.
