Retelling your story at every new clinic is exhausting. At PDC Health Hub your diabetes educator, dietitian, exercise physiologist, physiotherapist, podiatrist and pharmacist work from the same notes and talk to each other about your care. One NADC accredited allied health team, across Perth.

Your history, your results and what matters to you sit in one record. When you see the dietitian after your diabetes educator, they already know what changed and why, so the session starts where the last one finished instead of back at the beginning.

Advice from separate clinics can pull in different directions. One asks you to walk more, another adjusts your insulin, and nobody accounts for the other. Because we work as a single allied health unit, those decisions get made together rather than in isolation.

We see people across the Perth metropolitan area, so you can usually find a clinic near home or work rather than driving across town. Much of what we do is also available by telehealth, which matters if you live regionally or cannot easily get time off.

Chronic Disease Management plans, NDIS plans and DVA referrals each have their own rules about who you can see and how often. We check what you are eligible for before your first appointment, so you know the cost before you walk in.
