One referral, and the whole team can see the plan

PDC Health Hub is accredited by the National Association of Diabetes Centres as a secondary diabetes centre. One referral reaches diabetes education, dietetics, exercise physiology, physiotherapy, podiatry, pharmacy and our sleep clinic, across ten clinics in Perth and the Great Southern, plus telehealth.

Every card below is a link. Open one to send a referral, request a case conference, check what we treat and where our scope ends, see which funding pathways we work with, download our referrer resources, or find your nearest clinic.

Refer a patient

Send a referral online, or through your usual correspondence route. What we need from you, what we do not need, and what happens once it reaches our care team.

Case conferences

Our clinicians join multidisciplinary case conferences you organise, in the community and in residential aged care. Who attends, how to set one up, and the MBS items that apply on both sides.

What you get back

Referring into a multidisciplinary service should not mean losing sight of your patient.

A letter, not silence

Written correspondence goes back to you after the initial assessment and after any significant change in management, copied to anyone else named on the referral.

Named clinicians

Your patient sees a named clinician rather than a rotating roster, and we tell you who is covering when that clinician is away.

We say no when it is a no

If a referral sits outside our scope, or someone can be seen sooner or closer to home elsewhere, we tell you that rather than holding on to the referral.

Accredited, and scoped

PDC Health Hub is accredited by the National Association of Diabetes Centres as a secondary diabetes centre, and every clinician works to a written scope of practice.

Referral pathways

Four ways a person reaches us. The pathway decides the paperwork, not the clinical care.

From 1 July 2025 the GP Management Plan and Team Care Arrangements were replaced by a single GP Chronic Condition Management Plan: item 965 to prepare, item 967 to review, with 92029 and 92030 by video. The old three part allied health referral form is no longer required, so a referral letter is enough. Your patient can then claim up to five individual allied health services in a calendar year, or ten if they are Aboriginal or Torres Strait Islander. Plans written before 1 July 2025 keep working until 30 June 2027.
DVA works on the allied health treatment cycle. A D904 referral covers up to 12 sessions or one year, whichever ends first, and there is no limit on the number of cycles that are clinically necessary. We send a report at the end of each cycle so you can decide whether to issue the next referral. A veteran can use the treatment cycle with more than one allied health provider at the same time.
No GP referral is required for NDIS funded work, although clinical background always helps us start in the right place. We work with self managed, plan managed and agency managed participants, and write the assessment and progress reports that support a plan reassessment. Tell us the plan dates and how the plan is managed when you refer.
Support at Home replaced the Home Care Packages Program and Short Term Restorative Care on 1 November 2025, and the Commonwealth Home Support Programme continues alongside it. We see people in residential aged care and at home. Anyone can also be referred privately or self refer, with no plan and no waiting on paperwork.

Refer someone today

Send a referral through the online form, or call and talk it through with our care team first. If you are not sure which discipline to name, say so on the referral and we will work it out from the clinical detail.
