Funding and eligibility

The pathway your patient uses decides the paperwork and what they pay, not the clinical care they receive. The item numbers on this page are current, so you are not working from a 2024 printout.

Medicare, since 1 July 2025

The chronic disease management framework changed on 1 July 2025. If your practice software still prompts for a 721 and a 723, this is the section to read.

The plan: items 965 and 967

A GP prepares a GP Chronic Condition Management Plan under item 965 face to face, or 92029 by video, and reviews it under item 967 face to face, or 92030 by video. Items 392 and 393 are the equivalents for a prescribed medical practitioner.

What replaced the old items

Items 721, 723 and 732 ceased on 1 July 2025. A GP Management Plan or Team Care Arrangement already in place before that date keeps working until 30 June 2027, after which a GP Chronic Condition Management Plan is required. The old three part allied health referral form has gone with them: a referral letter is now enough, and the service still has to be recommended in the patient's plan.

The item numbers for our disciplines

Individual allied health services under the chronic condition management framework. Each requires at least 20 minutes of face to face time.

Diabetes education and dietetics

Item 10951 for a credentialled diabetes educator. Item 10954 for an accredited practising dietitian.

Movement and rehabilitation

Item 10953 for exercise physiology. Item 10960 for physiotherapy.

Podiatry

Item 10962 for a podiatrist, including diabetes foot assessment and the routine care that follows it.

Medication review

A Home Medicines Review is referred by the GP under MBS item 900 to an accredited pharmacist, and sits outside the five service allied health count.

Everything that is not Medicare

Four other ways your patient can be funded, and what each one asks of you.

DVA works on the allied health treatment cycle. A D904 referral covers up to 12 sessions or one year, whichever ends first, and there is no limit on the number of cycles that are clinically necessary. A report goes to you at the end of each cycle so you can decide whether to issue the next referral. A veteran can use the treatment cycle with more than one allied health provider at the same time.
No GP referral is required for NDIS funded work, although the clinical background always helps us start in the right place. We work with self managed, plan managed and agency managed participants, and write the assessment and progress reports that support a plan reassessment. Tell us the plan dates and how the plan is managed when you refer.
Support at Home replaced the Home Care Packages Program and Short Term Restorative Care on 1 November 2025, and the Commonwealth Home Support Programme continues alongside it. We also see residents in residential aged care, where the referral usually comes from the facility, the GP or the family rather than through a package.
Anyone can be referred privately or book themselves, with no plan and no item number involved. This is often the fastest route when the five service allied health count is already used up for the year, or when someone needs more contact than a plan will fund.

Still not sure which pathway applies?

Our care team checks eligibility before the first appointment and tells your patient what they will pay. If the pathway on the referral is not the right one, we say so before they book.
