A Credentialled Diabetes Educator (CDE) is a health professional credentialled by the Australian Diabetes Educators Association to specialise in diabetes care, on top of a primary qualification in nursing, pharmacy, dietetics or another health profession. Our CDEs work with people living with type 1 diabetes, type 2 diabetes, gestational diabetes and diabetes in pregnancy. Appointments cover insulin starting and dose adjustment, insulin pump and continuous glucose monitoring (CGM) training, hypoglycaemia and sick day planning, NDSS registration and supply, and fitting diabetes technology around your routine. An Accredited Practising Dietitian (APD) is a university qualified nutrition professional credentialled by Dietitians Australia, and the APD credential is the standard Medicare, the NDIS, DVA and private health insurers recognise. Musculoskeletal assessment, rehabilitation and pain.
Physiotherapists are AHPRA registered health practitioners who assess and treat movement, musculoskeletal injury and pain. Our physiotherapy team sees back and neck pain, shoulder, hip and knee problems, sporting injuries, post-operative rehabilitation, balance and falls prevention, and the joint, nerve and soft tissue changes that can come with long-term diabetes, including frozen shoulder and peripheral neuropathy. Pharmacists are AHPRA registered medicines experts. At PDC they sit inside the clinical team rather than behind a counter, so the person adjusting your insulin can also see everything else you take. Services include Home Medicines Reviews in your own home on GP referral, medication reconciliation after a hospital stay, help with side effects, interactions, timing and cost, dose administration aids, sick day and travel planning, and advice on injectable therapies and the devices that go with them. Obstructive sleep apnoea is common in people living with type 2 diabetes and often goes undiagnosed for years. Left untreated it affects daytime alertness, blood pressure, mood and glucose levels, and it makes everything else in a diabetes plan harder to hold.
