PDC diabetes educator in conversation with a client in a consult room

Care built around people, not appointments.

PDC Health Hub grew out of Perth Diabetes Care, started in East Victoria Park by Teresa and Jeff in 2015. We are now a multidisciplinary allied health team across Perth and the Great Southern, still built on one idea: care works better when the whole team works as one unit.

Our story

Teresa and Jeff opened Perth Diabetes Care in East Victoria Park in 2015, a single consulting room run by a Credentialled Diabetes Educator and a pharmacist. Everything since has been built outward from that room. Ten years on we are a multidisciplinary team, and the idea has not changed: one team, one record, one plan.

PHOTO NEEDED · Early days. An archive shot of the original East Victoria Park clinic, or Teresa and Jeff in the first consulting room. Grain and imperfection are fine, this one should look like history.
Teresa and Jeff started Perth Diabetes Care in East Victoria Park in 2015, a diabetes education service run by a Credentialled Diabetes Educator and a pharmacist. Everything since has been built outward from that one clinic.
PHOTO NEEDED · Landscape. Two or three clinicians from different disciplines around a screen or a set of notes, mid conversation. No white coats, no posing.
Your history, your results and what matters to you sit in one record. When you see the dietitian after your diabetes educator, they already know what changed and why, so the session starts where the last one finished.
PHOTO NEEDED · Landscape. A clinician working with a client, or a team meeting in progress. Something that reads as clinical standards rather than marketing.
PDC holds NADC Accredited Secondary Centre status, the national benchmark for diabetes centres. Every clinician is named on the site with their credentials, so you know who you are seeing before you book.
PHOTO NEEDED · Landscape. A peer support group or education session in progress, clients doing the talking and a clinician listening rather than presenting. Real people, permission on file.

Lived experience has been part of PDC from the beginning and still shapes how the service runs, through peer support and the people living with diabetes who tell us what actually fits a life. Clinical knowledge says what usually works. Lived experience says whether it will work on a Tuesday with three kids and a night shift.

Where you will find us

Ten clinics and telehealth. Nine sit across the Perth metropolitan area, north and south of the river, and one is in Kojonup in the Great Southern.

Not every discipline runs at every site. Each clinic page lists what is available there. Telehealth is available Australia wide and covers most of what we do, so distance need not decide your care.
