Credentialled Diabetes Educators supporting people living with type 1, type 2 and gestational diabetes, from first diagnosis to insulin pumps, CGM and closed-loop technology. Medicare, NDIS, DVA and private health accepted.
A Credentialled Diabetes Educator (CDE) is a health professional who has completed specialist post-graduate education in diabetes management and holds current credentialling with the Australian Diabetes Educators Association (ADEA). Our CDEs bring base professions in nursing, dietetics, pharmacy and podiatry together with deep, practical knowledge of diabetes self-management.

We support people living with type 1 diabetes, type 2 diabetes, LADA, MODY and gestational diabetes, whether you are newly diagnosed or years in and wanting a reset. Sessions cover blood glucose monitoring, insulin initiation and titration, carbohydrate counting and meal planning, sick day management and hypoglycaemia prevention.

PDC is a leader in diabetes technology education: continuous glucose monitoring (CGM) setup and review, insulin pump initiation, upgrades and troubleshooting, hybrid closed-loop systems and smart insulin pens. We also run group education programs, a dedicated clinic for teens and young adults, and support for diabetes in pregnancy.

Telehealth appointments are available across Western Australia. Medicare rebates apply with a GP Chronic Condition Management Plan, and we welcome NDIS participants, DVA card holders and people using private health insurance. Book online or call the clinic nearest you.

Your first session runs about 45–60 minutes. Your credentialled diabetes educator reviews your diabetes history, medications, blood glucose records and recent results, assesses your current knowledge and goals, then works with you on a personalised education plan you can actually live with. Your CDE liaises with your GP, endocrinologist and other treating clinicians so your education stays consistent with your broader medical care.
People living with type 1, type 2, LADA, MODY or gestational diabetes, whether you are newly diagnosed or years in and wanting a reset. We also support families and carers, run a dedicated clinic for teens and young adults, and provide pre-pregnancy planning and support for diabetes in pregnancy.
