Refer a patient

Three ways to send a referral, and what to put in it. Name every discipline you want on the one letter and we will do the scheduling.

Three ways to refer

All three reach the same care team. The form further down this page is the quickest, because it asks for everything we need in one pass.

A letter, your usual way

If your practice already has a route it uses for allied health referrals, keep using it. Address the letter to PDC Health Hub, name the discipline and the clinical question, and our care team will pick it up. Or ring (08) 6110 0570 and talk it through.
What to include

A phone call

Best when you are not sure which discipline it is, when something is urgent, or when the person has been seen here before and you want them back with the same clinician.

Refer online

Everything we need in one pass. Your referral is stored in Australia and read by the PDC care team, never emailed and never held by the website. Fields marked with an asterisk are required, and so is the consent tick at the end.

Your details

Patient details

We need a phone number or an email address so we can contact your patient to book.

Referral details

Please do not paste whole documents here. Send results and reports through your usual correspondence route, and we will match them to this referral.

Before you send it

Four things that decide whether the first appointment is treatment or history taking.

Tell your patient we will ring

We contact people by phone first. Let them know the referral has been sent and that it includes their clinical information, so the call is expected rather than a surprise.

Mark it if it is urgent

Every referral carries a date, so the date on its own tells us very little. If your patient needs to be seen sooner than routine, set the urgency on the form or write urgent on the letter, and say what is driving it: a pregnancy, a procedure, a discharge, or a foot at risk.

Who else is involved

Name the other clinicians in the picture, particularly the endocrinologist, so correspondence reaches everyone who needs it rather than only you.

Where they can get to

Ten clinics and telehealth. Tell us the suburb, or that telehealth is preferred, and we will place your patient accordingly.

Urgency, re-referrals and multiple services

How we handle the four situations referrers ask about most.

A routine referral is triaged by the care team and your patient is contacted to book. Tell us if there is a date that matters, such as a pregnancy, a planned procedure or a review appointment with you, and we will work backwards from it.
Write urgent on the referral and ring (08) 6110 0570 as well, so it is not sitting in a queue. Urgent here means a person at risk of harm before a routine appointment would come round, not simply a long wait. Anything acute belongs in an emergency department, not with us.
If the person has been seen here before, say so, and name the clinician if you want them back with the same person. Under Medicare a new referral is still needed once the services on the previous one have been used.
Name every discipline you want on the one referral letter. We schedule them together where we can, so your patient is not making four separate trips for appointments that could have been two.

Send the referral

The form on this page captures the clinical question, the funding pathway and any urgency in one pass. If you would rather talk it through first, ring the care team.
