About this service

The Medicare 45–49 Year Health Assessment (MBS items 701 to 707, depending on how long it takes) is a GP-led preventive health check designed for Australians aged 45–49 who are at risk of developing a chronic condition. The assessment is not a general check-up. It is a structured, evidence-based review focused on identifying modifiable risk factors for chronic disease and connecting the patient with appropriate preventive care and services.

PDC Health Hub supports GPs in coordinating follow-up allied health care for patients who receive a 45–49 assessment and are identified as requiring dietetic, exercise physiology, diabetes education, or podiatric input.

What the 45–49 assessment involves (GP-led)

  • Structured lifestyle risk factor review: diet, physical activity, smoking, alcohol
  • Blood pressure, BMI, and waist circumference measurement
  • Blood test review including glucose, lipids, and kidney function
  • Mental health and social wellbeing screening
  • Family history review for cardiovascular disease, diabetes, and cancer
  • Referral to allied health and preventive services as indicated

How PDC Health Hub can help

  • Patients referred by their GP following a 45–49 assessment are welcomed directly
  • No separate referral to individual clinicians is required for patients with a GP referral
  • We accept Medicare GPCCMP referrals for dietetics, exercise physiology, physiotherapy, podiatry, and diabetes education
  • Coordinated intake for patients with multiple identified risk factors

If your GP has conducted a 45–49 Year Health Assessment and referred you to allied health, please contact PDC Health Hub to book. Learn more about health screening at PDC Health Hub.

Common questions

Do I need a referral?
+

Most of our services can be booked without one. You do need a GP referral to claim a Medicare rebate, and a Home Medicines Review or Residential Medication Management Review always starts with a GP referral. NDIS, DVA and workers' compensation claims each have their own paperwork. If you are not sure, call us on (08) 6110 0570 and we will check before you book.

Can I claim a Medicare rebate?
+

If your GP has prepared a GP Chronic Condition Management Plan and referred you to us, Medicare pays a rebate for up to five eligible allied health visits a calendar year, shared across disciplines. Private health cover may also contribute, depending on your policy. We confirm what applies to you when you book.

Can I be seen by telehealth?
+

Many of our services can be delivered by secure video call. Where a service needs a hands-on assessment, such as a foot or physiotherapy examination, we will book you in person at one of our ten clinics.

What should I bring to my first appointment?
+

Your referral or care plan if you have one, a list of your current medicines, any recent test results, and your Medicare, DVA or NDIS details. If you use a glucose meter, CGM or insulin pump, bring that too.

Can someone come with me?
+

Yes. A family member, carer, interpreter or support worker is welcome at any appointment. If you need an interpreter, let us know when you book.
