Diabetes Risk Assessment

Structured assessment of your risk of developing type 2 diabetes using the AUSDRISK tool, with results, personalised advice, and referral to diabetes education or allied health where indicated.

About this service

Type 2 diabetes develops over years, often preceded by a period of prediabetes during which blood glucose levels are elevated but not yet in the range diagnostic of diabetes. Identifying risk early creates a window of opportunity for lifestyle intervention that has been shown to delay or prevent the onset of type 2 diabetes. The Australian Type 2 Diabetes Risk Assessment (AUSDRISK) tool is a validated, evidence-based screening instrument used nationally to identify people at elevated risk.

At PDC Health Hub, diabetes risk assessment is available to anyone concerned about their risk, or on referral from a GP or other health professional.

What a diabetes risk assessment includes

  • AUSDRISK questionnaire completion and score interpretation
  • Review of relevant blood test results if available (fasting glucose, HbA1c, OGTT)
  • Discussion of modifiable and non-modifiable risk factors
  • Explanation of what prediabetes means and does not mean
  • Personalised lifestyle recommendations for risk reduction
  • Referral to diabetes education, dietetics, or exercise physiology where indicated

What to expect

  • A brief, structured assessment, typically 15 to 20 minutes
  • Clear, plain-language feedback on your individual risk level
  • Written summary of recommendations provided to you and your GP
  • Warm referral to relevant PDC Health Hub clinicians if follow-up is indicated

Preparation tips

Bring any recent blood test results from your GP if available. Knowing your weight, height, waist circumference, and family history of diabetes will help your clinician complete the assessment accurately.

Diabetes risk assessment is available at PDC Health Hub. Referral fees may apply depending on the treating clinician. Learn more about health screening at PDC Health Hub.

Common questions

Do I need a referral?
+

Most of our services can be booked without one. You do need a GP referral to claim a Medicare rebate, and a Home Medicines Review or Residential Medication Management Review always starts with a GP referral. NDIS, DVA and workers' compensation claims each have their own paperwork. If you are not sure, call us on (08) 6110 0570 and we will check before you book.

Can I claim a Medicare rebate?
+

If your GP has prepared a GP Chronic Condition Management Plan and referred you to us, Medicare pays a rebate for up to five eligible allied health visits a calendar year, shared across disciplines. Private health cover may also contribute, depending on your policy. We confirm what applies to you when you book.

Can I be seen by telehealth?
+

Many of our services can be delivered by secure video call. Where a service needs a hands-on assessment, such as a foot or physiotherapy examination, we will book you in person at one of our ten clinics.

What should I bring to my first appointment?
+

Your referral or care plan if you have one, a list of your current medicines, any recent test results, and your Medicare, DVA or NDIS details. If you use a glucose meter, CGM or insulin pump, bring that too.

Can someone come with me?
+

Yes. A family member, carer, interpreter or support worker is welcome at any appointment. If you need an interpreter, let us know when you book.
