A credentialled pharmacist visits you at home, goes through every medicine you take and sends a report to your doctor. It is free for eligible people living at home with a Medicare or DVA card, and it starts with a referral from your GP.
A Home Medicines Review is a one-to-one visit from a credentialled pharmacist in your own home. We look at everything you take, how you take it and how you store it, then work with your doctor so your medicines are as safe and as useful as they can be. The Australian Government funds the pharmacist's visit, so there is nothing to pay.

People taking several medicines, people just home from hospital, and anyone whose medicines or health have recently changed. It also helps when side effects, a confusing routine or hard-to-open packs are getting in the way.

Prescription, over-the-counter and complementary medicines, including vitamins, creams, eye drops, inhalers and injections. We check for interactions, doubling up, doses that no longer fit, and how easy each one is to use.

We send a written report to your doctor with what we found and what we suggest. Your doctor talks it through with you and agrees a medication plan, and we can come back for a follow-up visit to see how the changes are going.
Four steps, from your doctor's referral to a plan you both agree on. You only need to be home for the visit itself.
Your GP, or an eligible specialist, decides whether a review would help and sends a referral to PDC Health Hub.
Once the referral arrives, our pharmacist calls to find a time that suits you and explains what to have ready.
The pharmacist sits down with you, and a family member or carer if you like, and goes through every medicine you have.
We send the report to your doctor. You then see your doctor to talk it through and agree any changes to your medicines.
The rules come from the Australian Government's Home Medicines Review programme. If you are not sure whether you qualify, ask your doctor or give us a call.
You need to live in the community, not in hospital or a residential aged care home, and hold a current Medicare or DVA card. Your doctor must agree that you are at risk of problems with your medicines, for example after a hospital stay, a change to your medicines or a change in your health.
Your GP can refer you, and so can a specialist physician, psychiatrist, pain medicine specialist or palliative medicine specialist. You cannot refer yourself, but you can ask your doctor whether a review is right for you.
The pharmacist's visit and report are funded by the Australian Government, so there is nothing to pay. Your GP claims their part of the review through Medicare, so ask your practice whether your follow-up appointment is bulk billed.
Usually once every two years. Your doctor can refer you again sooner if your health or your medicines change significantly.
After the first review, up to two follow-up visits can be arranged within nine months to check how the changes are working for you.
Face to face, in your home. Reviews are not done by phone or video, and another place can only be used in special circumstances, such as for cultural or safety reasons.
Call us and we will explain how the review works and what to ask your doctor.

Yes. A Home Medicines Review needs a referral from your GP or an eligible specialist. Ask at your next appointment and mention PDC Health Hub.
Put out every medicine you use, including prescription and over-the-counter medicines, vitamins, herbal products, creams, drops, inhalers and injections, along with any medicine lists, dose packs or recent hospital letters.
Usually about an hour. Our pharmacist will give you a better idea when they call to book the visit.
Yes. Many people find it helps to have someone with them, especially the person who helps with their medicines.
No. The programme requires the review to happen face to face in your home, apart from special circumstances approved in advance.
Not a Home Medicines Review. People living in residential aged care have a different medication review, arranged through the aged care home and their GP.
Ask your GP for a referral and mention PDC Health Hub. Once it arrives, our pharmacist will call you to book a time to visit. For anything else, book online or call our care team.
