Aged Care Services

Allied health, pharmacy and diabetes care for older Australians at home, in clinic, and under contract to residential aged care homes: dietetics and menu review, physiotherapy, exercise physiology, podiatry, diabetes education and medication reviews. Medicare, Support at Home, CHSP, DVA and NDIS pathways.

How we support older Australians

Getting older often means more medicines, more appointments and more to keep track of. Our pharmacists, podiatrists, physiotherapists, exercise physiologists, dietitians and credentialled diabetes educators work as one team with your GP, your aged care provider and your family, so care stays joined up and you stay well, mobile and safe with your medicines.

At home

Home Medicines Reviews take place in your own home, and many of our allied health services can come to you. Seeing how you live day to day helps us spot falls risks, medicine problems and foot care needs early.

In aged care homes

Homes can contract any of our disciplines for scheduled on-site visits: dietetics and menu review, physiotherapy, exercise physiology, podiatry, diabetes education and medication review. We write into your records and teach your care staff as we go.

In our clinics

Our clinics bring every discipline under one roof, with supervised strength and balance work in our own gym and pharmacists working beside the clinical team. Family members and carers are welcome at appointments.

Allied health for residential aged care homes

Aged care homes can contract any of our disciplines, not only diabetes care. One service agreement covers scheduled on-site visit days, notes and reports written for your own records, and short education sessions for care staff. Dietetics, physiotherapy, exercise physiology and podiatry are the services homes ask for most.

Dietetics, menu and mealtime review

Accredited Practising Dietitians for unplanned weight loss and malnutrition, texture-modified diets and IDDSI levels, high-risk and enteral feeding advice, and menu and mealtime reviews that support your food and nutrition evidence under Standard 6.

Physiotherapy

Mobility and transfer assessments, pain and musculoskeletal management, falls prevention, and manual handling and equipment advice, with simple programmes care staff can carry on between visits.

Exercise physiology

Group and individual strength and balance programmes that keep residents moving, including reablement after a fall, an illness or a hospital stay, and seated options for residents with limited mobility.

Podiatry

Routine nail and skin care on a visit schedule that suits the home, diabetes foot assessments with risk stratification, footwear and offloading advice, and escalation when a wound needs faster attention.

Diabetes education

Credentialled diabetes educators for insulin and glucose monitoring reviews, hypoglycaemia management plans your staff can follow, device and sensor support, and education for registered and care staff.

Pharmacy and medicines

Residential Medication Management Reviews with the resident's GP, quality use of medicines support, dose administration aids, and medication education sessions for care staff.

Paying for aged care services

Most older Australians can use more than one funding pathway, and the right one depends on the service and where you live. Open each panel for how Medicare, Support at Home, CHSP, DVA, NDIS and private appointments work at PDC Health Hub. Our team confirms your entitlements and any out-of-pocket cost before your first visit.

Medicare
+

With a GP Chronic Condition Management Plan, Medicare rebates apply to up to five individual allied health visits each calendar year across physiotherapy, podiatry, dietetics, exercise physiology and diabetes education. A Home Medicines Review is free for eligible people with a Medicare or DVA card and a referral from their GP.

Support at Home
+

Support at Home replaced Home Care Packages on 1 November 2025. Allied health can be part of your Support at Home services where it meets an assessed need, and clinical supports such as physiotherapy carry no participant contribution. Ask your provider or care partner to arrange visits with us, or call us and we will contact them with you.

Commonwealth Home Support Programme (CHSP)
+

CHSP continues for people who need a small amount of help to keep living at home, and moves into Support at Home no earlier than 1 July 2027. If you receive CHSP services, call us and we will work out with you and your provider what can be arranged.

DVA
+

Veteran Gold Card holders, and White Card holders for their accepted conditions, can see our allied health team with a referral from their GP. Home Medicines Reviews are also available to DVA card holders.

NDIS
+

The NDIS is open to people who apply before they turn 65. If you joined the scheme before 65, you can keep using your plan for our allied health services. PDC Health Hub is a registered NDIS provider.

Private appointments
+

No referral is needed for private allied health appointments, and private health insurance rebates may apply depending on your cover.

Aged care homes and providers
+

Homes usually fund contracted allied health visits themselves under a service agreement with us, and we invoice the home rather than the resident. Residents can also use DVA, and Medicare rebates may apply where a resident's multidisciplinary care plan recommends allied health. Call us and we will set out visit days, reporting and staff education in one agreement.

Frequently asked questions

Common questions from older adults, families and carers about aged care services at PDC Health Hub.

Do I need a referral?
+

Not for private allied health appointments. You need a GP referral for Medicare rebates, for DVA-funded allied health and for a Home Medicines Review.

What is a Home Medicines Review?
+

A credentialled pharmacist visits you at home, goes through every medicine you take, including over-the-counter and complementary medicines, and sends a report to your GP. It needs a GP referral and is free for eligible people living at home with a Medicare or DVA card.

Can you visit me at home?
+

A Home Medicines Review always takes place in your home, and many of our allied health services can be delivered at home too. Call us to check what is available in your area.

Do you work with residential aged care homes?
+

Yes, and not only for diabetes care. Homes contract us for dietetics and menu reviews, physiotherapy, exercise physiology, podiatry, diabetes education and medication review, on a visit schedule that suits the roster. One service agreement covers the lot, and our clinicians write notes and reports for your own records. Call us or email admin@pdchealthhub.com.au to talk it through.

Can Support at Home pay for allied health?
+

Yes, where it meets an assessed need. Clinical supports such as physiotherapy have no participant contribution under Support at Home. Your provider or care partner arranges the service with us.

Do you review menus and mealtimes for aged care homes?
+

Yes. Our Accredited Practising Dietitians review menus and the mealtime experience, and report back in a form you can put straight into your food and nutrition evidence. We also see residents individually for unplanned weight loss, malnutrition, texture-modified diets and enteral feeding.

Can a family member or carer book for someone?
+

Yes. Family members and carers can call us to book, and are welcome at appointments with the person's consent.
