A Residential Medication Management Review (RMMR) is a government-funded review of the medicines of a permanent resident of an aged care home, carried out by an accredited pharmacist working with the resident's GP.
The resident's GP requests the review. The pharmacist looks at the medication chart and clinical notes, talks with the resident and care staff where possible, and writes recommendations for the GP, who decides on any changes.
People in residential aged care often take many medicines and face a higher risk of falls, confusion and other side effects, so regular reviews support safer use of medicines. RMMRs are funded through the Pharmacy Programs Administrator and residents do not pay for them. Aged care homes taking part in the Aged Care On-site Pharmacist measure do not use the RMMR programme.
Our clinicians can explain how this applies to you and help with the next step, whether that is a referral, a device, funding or a plan the rest of the team can see.
