A Home Medicines Review (HMR) is a government-funded review of all your medicines, carried out in your home by an accredited pharmacist after a referral from your doctor. It is provided at no cost to you.
An HMR suits people living at home who are at risk of problems with their medicines, for example people taking several medicines or not reaching their treatment goals. Your GP can refer you, as can some specialists, including physicians, psychiatrists, and pain and palliative medicine specialists.
The pharmacist visits you at home, looks at everything you take, including over-the-counter and complementary medicines, and talks through how you use them. They then send a report to your doctor, who agrees a medication management plan with you.
An HMR is generally available once every 24 months, or sooner if your doctor sees a clinical need, such as a significant change in your health or medicines. People living in residential aged care have a separate review, the Residential Medication Management Review.
Our clinicians can explain how this applies to you and help with the next step, whether that is a referral, a device, funding or a plan the rest of the team can see.
