An accredited pharmacist is a pharmacist with extra training and a credential to carry out medication management reviews, such as Home Medicines Reviews and Residential Medication Management Reviews. The role is now officially called a credentialed pharmacist.
After the former accrediting body closed in 2022, new national standards were developed, and pharmacists moved from accreditation to a Medication Management Review (MMR) credential. Transitional arrangements for Home Medicines Reviews and Residential Medication Management Reviews ended on 31 December 2025. Pharmacists who work on-site in residential aged care homes need a separate Aged Care Onsite Pharmacist (ACOP) credential.
A Home Medicines Review is carried out by a credentialed pharmacist, face to face in your home, after a referral from your GP or certain specialists. It suits people living in the community who are at risk of problems with their medicines. The pharmacist looks at all your medicines and how you take them, and the findings feed into a medication management plan with your doctor. You need a current Medicare or DVA card, and people in hospital or residential aged care are not eligible for this type of review.
Our clinicians can explain how this applies to you and help with the next step, whether that is a referral, a device, funding or a plan the rest of the team can see.
