Support from people who live with diabetes too

PDC Collective and PDC Mums & Bumps bring people living with diabetes together with others who share the lived experience. Both are free, both are confidential, and both are open to family members and carers as well.

Two ways to connect

One is a community group for anyone living with any type of diabetes. The other is for pregnancy. Both are free, and you do not need a referral to come along.

PDC Collective

Our community group for people living with diabetes. Come along to connect, share what actually works day to day, and learn alongside people who understand it from the inside. Open to every type of diabetes, and to family members and carers.

PDC Mums & Bumps, with you every step of the way

PDC Mums & Bumps

Support through pregnancy for women with gestational diabetes, and for women with type 1 or type 2 diabetes who are pregnant or planning a pregnancy. It sits alongside our diabetes in pregnancy clinical services rather than replacing them.

What peer support actually gives you

Clinical care and lived experience do different jobs. Our clinicians bring the evidence. The group brings the part that only comes from managing diabetes yourself, every day, for years.

Lived experience

Time with people who manage diabetes themselves rather than only treat it. The practical detail that never makes it into a consultation tends to come up here.

Free and confidential

There is no cost to attend either programme, and what people share in the group is kept in confidence by everyone in it.

Every type of diabetes

Type 1, type 2, gestational and the less common types, however you manage it: tablets, injections, a pump or a hybrid closed loop system.

A way to give something back

How to join

Four steps, and you can stop at any one of them. Pick a step to see what actually happens.

PHOTO NEEDED · Landscape 16:9. A hand holding a phone mid call, PDC reception soft in the background. No screen detail legible.
There is no referral and no form. Ring (08) 6110 0570, email admin@pdchealthhub.com.au, or mention it to whichever clinician you are seeing next. You do not have to explain why, and telling us you are interested commits you to nothing.
PHOTO NEEDED · Landscape 16:9. A community room set up before a session: chairs in a loose circle, tea and biscuits on a side table, nobody in shot yet.

Our team comes back to you with the next PDC Collective or Mums & Bumps session: when it is, where it is, roughly how many people come and how long it runs. If that date does not suit, we will tell you when the one after it is.
PHOTO NEEDED · Landscape 16:9. Two or three people talking easily at a session, cups in hand, mid laugh. Signed consent required from everyone identifiable.

Turn up, take a cup of tea, and listen if that is all you feel like doing. Nobody is asked to speak. Come once and never come back, or come to every session; both are completely fine and neither is remarked on.
PHOTO NEEDED · Landscape 16:9. A couple sitting together at a session, one listening while the other speaks. Signed consent required.
Diabetes is managed by households, not by individuals on their own. A partner, a parent, an adult child or a carer is welcome to come with you, and often takes as much from the session as the person living with diabetes does.
