Manual Therapy: Joint Mobilisation and Manipulation

Hands-on joint mobilisation and manipulation from a Perth physiotherapist, used alongside exercise and advice for back pain, neck pain and joint stiffness.

#40ab4b

About this service

Manual therapy is skilled, hands-on treatment where a physiotherapist moves the joints and soft tissues of your spine or limbs. Mobilisation uses gentle to firm, repeated movements within the joint's range. Manipulation (spinal manipulative therapy) is a quick, small thrust at the end of the range. It is most often offered for back pain, neck pain and joint stiffness, and it is always one option among several, not a requirement of care.

Manual therapy is used alongside exercise and education rather than on its own. Some people find it eases pain and stiffness and makes movement more comfortable for a time, and others notice little change, so we review whether it is helping.

Most side effects are mild and short-lived, such as muscle soreness or a brief increase in pain.1 Serious complications from neck manipulation, such as injury to the arteries of the neck, are rare, so your physiotherapist asks careful questions, adapts the examination and weighs up whether a neck thrust technique is appropriate at all.3,4 You will be told the benefits, risks and alternatives, such as gentler mobilisation, and your consent is sought before any neck manipulation. You can say no at any point.5

What is involved

  • An assessment of your problem, including a history and examination, with screening for features that need medical review before any hands-on treatment.
  • A discussion of your options, the possible benefits and the risks, so that you can give or decline consent for each technique.
  • Joint mobilisation or, where appropriate and agreed, manipulation, with your response checked as we go.
  • Exercise and advice in the same visit, so that hands-on treatment supports a longer-term plan.
  • A review of progress against your goals, with hands-on treatment reduced as your own strategies take over.

Common questions

Do I need a referral?
+

Most of our services can be booked without one. You do need a GP referral to claim a Medicare rebate, and a Home Medicines Review or Residential Medication Management Review always starts with a GP referral. NDIS, DVA and workers' compensation claims each have their own paperwork. If you are not sure, call us on (08) 6110 0570 and we will check before you book.

Can I claim a Medicare rebate?
+

If your GP has prepared a GP Chronic Condition Management Plan and referred you to us, Medicare pays a rebate for up to five eligible allied health visits a calendar year, shared across disciplines. Private health cover may also contribute, depending on your policy. We confirm what applies to you when you book.

Can I be seen by telehealth?
+

Many of our services can be delivered by secure video call. Where a service needs a hands-on assessment, such as a foot or physiotherapy examination, we will book you in person at one of our ten clinics.

What should I bring to my first appointment?
+

Your referral or care plan if you have one, a list of your current medicines, any recent test results, and your Medicare, DVA or NDIS details. If you use a glucose meter, CGM or insulin pump, bring that too.

Can someone come with me?
+

Yes. A family member, carer, interpreter or support worker is welcome at any appointment. If you need an interpreter, let us know when you book.
