Many of our allied health services attract a Medicare rebate when your GP has prepared a GP Chronic Condition Management Plan and referred you to us.

In plain English
Medicare is not an accreditation, but it is a recognition that matters. Each of our allied health clinicians who bills Medicare holds their own Medicare provider number, which they can only get with the right professional credential.
From 1 July 2025 the GP Chronic Condition Management Plan replaced the earlier GP Management Plan and Team Care Arrangements. With a plan in place and a referral from your GP, Medicare pays a rebate for up to five individual allied health services each calendar year, shared across the disciplines you need.
The items are 10951 for diabetes education, 10953 for exercise physiology, 10954 for dietetics, 10960 for physiotherapy and 10962 for podiatry. A referral letter from your GP is enough; the old three part referral form is no longer required. The rebate may not cover the whole fee, so ask us about any gap when you book.
Keeping it current
Medicare rules and item numbers change. We checked the items on this page against MBS Online in September 2026.
If you are unsure whether you are eligible, ask your GP or call us and we will talk it through.
