GP & Nurse event: referral pathways and options at PDC

Join us for an information evening, specific for Practice Nurses who are involved in Care Plans and referring patients for allied health support (Diabetes, Podiatry, Exercise Physiology, Dietetics). There is an abundance of referral options and pathways for patients requiring allied health support, so we're delivering an information evening that will cover the following:

• Making the most of Chronic Disease Management Plans for your patients
• Allied Health Group Referrals - a bonus referral option for your patients with type 2 diabetes
• Discussion of the various billing codes available
• Multidisciplinary case conferences - what they are and how to make them work for you and your patients

Light refreshments will be provided, and you will have the opportunity to tour the PDC Health Hub HQ in East Victoria Park; WA's largest private community-based diabetes clinic.
